Medication Forms

The RSU 71 Policy regarding a student taking medications at school:

A. All medication is to be delivered to the school by an adult

B. All prescription drugs must have:

a. The original Pharmacy bottle with the Student's name and date of birth on the label.

b. A parent signature on Medication Order Sheet

c. A signed order from the prescribing practitioner

C. If a child brings medication to school or it does not meet the above conditions, the parent will

be contacted by phone. The medication will not be administered by school staff.
